By submitting this interest form, I acknowledge and agree to the following policies for the Stage Door Dance Company 2026–2027 season:

• I understand that participation in dance training and auditions involves inherent physical risks, and I voluntarily assume responsibility for any injury or medical emergency that may occur. I authorize Stage Door Dance Studio staff to seek appropriate medical treatment if necessary and release Stage Door Dance Studio from liability related to medical expenses or injuries sustained during participation.
• I understand that company placement decisions are made solely at the discretion of Kelly Colgan Thaxton and Stage Door Dance Studio. Previous placement, training history, personal preference, or outside recommendations do not guarantee placement within a specific Company, group, or level.
• I understand that acceptance into SDDC requires a full commitment to all financial and training obligations, including tuition, required classes, competition fees, costumes, choreography fees, travel expenses, and any additional studio costs outlined following placement.
• I understand that consistent attendance, punctuality, preparation, teamwork, and positive behavior are required to maintain placement within the Company. Failure to meet these expectations may result in reassignment, probation, or removal from Company programs and opportunities.
• I acknowledge that Stage Door Dance Studio reserves the right to modify Company structures, schedules, class requirements, choreography, competitions, conventions, performances, and other opportunities as needed throughout the season.
• By submitting this form, I confirm that I have read, understood, and agreed to all terms and policies outlined above.
